5/26/2026	2026-27 UIL PHYSICAL/MEDICAL HISTORY FORMS - Please click the link for the 2026-27 UIL Physical/Medical History Forms. These forms must be completed before the first day of practice or school, whichever comes first. The forms cannot be dated before May 1, 2026. For all 9th-12th graders, please email a copy of the forms as a pdf to the HPISD Head Athletic Trainer, Christal Benson, bensonc@hpisd.org and the Assistant Athletic Trainer, Dan Riegle, riegled@hpisd.org. For all 7th-8th graders, please email the forms as a pdf to the Assistant Athletic Trainer, Jordan Graham, grahamj@hpisd.org. No photos are allowed. Go Scots! Link
5/3/2026	2026-27 RANK ONE FORMS - The Rank One Forms must be completed for all incoming 7th-12th athletes before the first day of practice or first day of school, whichever comes first. To complete the forms, the parents must have their child's student id number.
