2026-27 UIL PHYSICAL/MEDICAL HISTORY FORMS - Please click the link for the 2026-27 UIL Physical/Medical History Forms. These forms must be completed before the first day of practice or school, whichever comes first. The forms cannot be dated before May 1, 2026.
